You can find the required health forms for the 2014-2015 school year on the Parent Portal under Downloads - General - Healh Forms or simply by clicking here. Please return completed forms to the nurse's office prior to the first day of school, August 20, 2014. If you need additional blank forms you may contact the appropriate divisional administrative assistant.
Authorization to Administer Medication & Physician’s Certification:
Parents of all new students and returning students 7th-12th must complete the Authorization to Administer Medication and Physician’s Certification. Completed forms must be submitted prior to any participation in school sports including practice.
Parents of returning PK-6th students must complete the Authorization to Administer Medication omitting the Physician’s Certification portion.
Every student enrolled is required to be appropriately immunized and have records on file at our school. New students must have proper documentation submitted to the nurse’s office by August 18, 2014. Returning students are required to submit any new vaccinations so that we may update all student files.
Consent to Administer Medication:
This form must be completed if prescription medication is required during school hours.
